I'm having trouble making any sense out of all this, perhaps partly
because I have no idea what distro/apps/versions you are using.

The form was immediately available from my first hit on Google, and it
downloaded with no problem in Linux.  I also had no trouble filling it
in, and printing it, under Linux.

I'll take your word for it that it also works in Windows, but I really
couldn't care less.  8-)
