I'm not familliar with the tool you used to test the drive, but I can tell 
what the output means.  The disk is reporting that it is uninitialized.  If 
this is not  true (eg, you did properly install and format the drive), there 
maybe a flaw in the RAID controller, or it may be simply a bad drive itself.

You should be able to format the drive outside of the array to test it.
