Could anyone give me any comments or suggestions on this?

I am looking for someone to set up an online form (users input
information) that automatically collates the information and sets up a
webpage displaying a summary for all users.  For instance, 50 people
would input how many years they've been working, how much they make and
where they work.  The site would take this information from everyone and
display the summary of the average salary, etc.  Of course the data
would be more complicated and there would be more input data, but you
get the idea (I hope).

Would this be something that Apache, PHP, MySQL would be best to
accomplish?  Is this terribly complicated?  Can anyone recommend someone
that can do something like this and at what appoximate cost?
